Pag-IBIG Loyalty Card Requirements and Eligibility
Before you proceed with your application, make sure that you meet the eligibility criteria and requirements for owning a Pag-IBIG Loyalty Card.
You may apply for a Pag-IBIG Loyalty Card as long as you meet the following criteria:
- You must have a Pag-IBIG Membership ID (MID) number.
- You must be an active Pag-IBIG member or MP2 retiree member under Circular No. 407 who opted for an annual dividend payout.
If you meet the aforementioned eligibility criteria, you can apply for a Pag-IBIG Loyalty Card by providing the following requirements:
1. Duly accomplished Loyalty Card Plus application form (HQP-PFF-108) that you can download from the Pag-IBIG website.
2. Valid ID acceptable to the Pag-IBIG Fund (1 original and 1 photocopy).
3. ₱125 application fee.

Step-by-Step Guide on How to Apply for a Pag-IBIG Loyalty Card Plus
At the moment, applications for the Pag-IBIG Loyalty Card Plus (LCP) can only be done in person at selected Pag-IBIG branches with LCP kiosks. Online application for the Pag-IBIG Loyalty Card is not yet available. You can use this branch locator to see a list of Pag-IBIG branches nationwide with LCP kiosks.

Step 1: Download and Print the Pag-IBIG Loyalty Card Plus Application Form
The first thing you should do is to download the Loyalty Card Plus application form. After downloading the form, open the document using a free PDF reader such as Adobe Acrobat. If you are using Chrome on a desktop, the form will automatically open in a new tab so you don’t need to use a PDF reader.
You can also get the Pag-IBIG Loyalty Card Plus application form at any Pag-IBIG branch nearest you. Take note that you only need one (1) copy of the form.
Step 2: Fill Out the Pag-IBIG Loyalty Card Plus Application Form
- The next step is to fill out the Loyalty Card Plus application form.
- Fill out the application form in one (1) copy only.
- Write all entries in BLOCK or CAPITAL LETTERS only.
- Fields marked with an asterisk (*) are required.
- Write N/A if the information is not applicable.
Don’t forget to write your 12-digit Pag-IBIG Membership ID (MID) number and the issuing partner bank (e.g. UnionBank, AUB) on the upper-right portion of the form.
Step 3: Submit Application Form and Valid ID to Pag-IBIG
After completing the LCP application form, go to the nearest Pag-IBIG branch and submit it together with your valid ID. Pag-IBIG branches are open from 8:00 AM to 5:00 PM, Mondays to Fridays (except holidays).
The following is a list of valid IDs that are accepted by the Pag-IBIG Fund:
- Passport
- Philippine National ID (PhilID)
- Unified Multi-Purpose ID (UMID)
- Government Service Insurance System (GSIS) e-Card
- Social Security System (SSS) Card
- Driver’s License
- Professional Regulation Commission (PRC) ID
- National Bureau of Investigation (NBI) Clearance
- Police Clearance
- Postal ID
- Voter’s ID
- Barangay Certification, Barangay ID, or similar documents bearing a picture of the member
- Senior Citizen Card
- Integrated Bar of the Philippines (IBP) ID
- Overseas Workers Welfare Administration (OWWA) ID
- Overseas Filipino Worker ID
- Seaman’s Book or Seafarer’s Identification and Record Book (SIRB)
- Alien Certification of Registration or Immigrant Certificate of Registration
- Government Office and GOCC ID (e.g. AFP ID, Pag-IBIG Fund Loyalty Card)
- Certification from the National Council for the Welfare of Disabled Persons (NCWDP)
- Department of Social Welfare and Development (DSWD) Certification
- Company ID issued by private entities or institutions registered with, supervised, or regulated by the BSP, SEC, or IC
Step 4: Pay the Card Application Fee
Next, pay the ₱125 application fee to the bank representative. Take note that you should only pay the fee to the bank personnel assigned to the Loyalty Card Plus kiosk at the Pag-IBIG branch.
Step 5: Have Your Picture and Fingerprints Taken
The Pag-IBIG Loyalty Card Plus representative will take your picture, scan your fingerprints, and ask you to sign on the digital tablet. The representative will then request you to validate your information as shown on the computer monitor and will proceed with the onsite card personalization.
Step 6: Claim Your Pag-IBIG Loyalty Card
In less than an hour, you can claim your brand-new Pag-IBIG Loyalty Card Plus. You will receive your card along with the PIN mailer from the bank representative.
